Super Canine is committed to protecting the personal information of our clients, members, customers and other individuals we deal with. We recognise that individuals have a right to control how their personal information is handled. Providing personal information is an act of trust that we take seriously.
Personal information is information about you, such as your name, contact details and records of your dealings with us. Personal information may, in some cases, include sensitive information, such as your health information.

2. Collection of Confidential Client Information
Confidential client information includes any individually identifiable information regarding a client’s medical history, mental or physical condition, treatment, and related records. This may also include information relating to family members.

Physical medical and psychiatric records including electronic, paper, photo, video, diagnostic and therapeutic reports, laboratory and pathology samples

We only collect personal information by lawful and fair means. We generally collect personal information directly from you unless it is unreasonable or impracticable to do so.
In some cases, we may obtain contact details of potential supporters from third parties that hold such information.
When collecting personal information from children and young people under 15 years old, we seek the permission of a parent or guardian first.

Depending on the purpose for which your personal information was collected, and only where permitted by privacy laws, we may disclose that information to:

5. Accuracy and Correction

If we believe information we hold is inaccurate, out-of-date, incomplete, irrelevant or misleading, we will take reasonable steps to correct it.
You may request corrections to your personal information. We will respond within a reasonable time. If we do not agree with the requested correction, we will provide a written notice outlining:

6. Security of Your Information
We hold personal information in both paper-based and electronic files.

We take reasonable steps to protect personal information from misuse, interference, loss, unauthorised access, modification or disclosure. We are obliged to respect the confidentiality of all personal information held by us.
When personal information is no longer required for a lawful purpose (including when we are no longer legally required to retain records), we ensure it is destroyed or de-identified.
All information collected and stored will be retained for a minimum of 7 years.
